NinePatch Whole Person Care is a highly integrated care and services coordination management platform that provides tools to address every step of a Whole Person Health practice model.  Starting with a robust client identification and consent management process, services needs are assessed and identified, closed-loop referrals are made to partners in the resources network, clients are enrolled in relevant programs offered by the community, and case follow-ups and reminders ensure that individuals and families get the support they need all the way through.  Lastly, robust reporting capabilities allow service providers and payers to measure the outcomes and impact of their work.
